Factors influencing the failure to report incidents and adverse events at an accredited third-level institution in Bogotá

Resumen

El objetivo de este estudio es determinar los factores que contribuyen a la omisión del reporte del incidente o evento adverso, por parte del personal de salud, y caracterizar por unidad funcional la omisión del reporte para orientar las estrategias de intervención y mejorar la cultura de seguridad. El estudio es descriptivo-analítico. Para lo anterior, se tomó como población objeto al personal de salud de una institución acreditada de tercer nivel en Bogotá. El número total de encuestas se determinó por medio de la metodología de muestreo estadístico, que permite la recolección de los datos a partir de la aplicación de una encuesta al personal asistencial y administrativo de la institución de salud, a través de la cual se determinaron los factores que influyen en la omisión del reporte de incidentes y eventos adversos. Dentro de los resultados más relevantes se encontró que el formato de reporte es demasiado complicado, que no se retroalimenta al personal cada vez que se analizan los incidentes y eventos adversos reportados y que el personal se preocupa por las medidas disciplinarias que la institución pueda tomar al momento de analizar los casos.

Abstract

This study aims to determine the factors that contribute to the failure to report an incident or adverse event by health personnel, and to characterize the failure to report by functional units in order to guide intervention strategies and to improve safety culture. It is a descriptive-analytical study. To the effect, health personnel from an accredited third-level institution in Bogotá was selected as target population. The total number of surveys was determined by statistical sampling methodology this allowed data collection based on the application of a survey to healthcare and administrative staff at the health institution, which helped to determine factors influencing the failure to report incidents and adverse events. Among the most relevant results, it was found that the reporting form is too complicated that there is no feedback to the staff when reported incidents and adverse events are analyzed and that the staff is worried about disciplinary measures the institution might take when analyzing cases.
