Resumen

La Doble Carga de la Malnutrición (DCM) se refiere a la coexistencia o simultaneidad de personas con desnutrición (bajo peso) y con sobre nutrición (exceso de peso) en un mismo grupo poblacional. Es un problema de salud pública tanto en países en desarrollo (PED) como desarrollados y ha sido considerado como un fenómeno multicausal, al cual se le ha asociado fuertemente con la transición nutricional y a la transición rural-urbana. Se planteó como hipótesis, que la prevalencia de DCM está asociada con la distribución de la riqueza de los hogares y con la inequidad en el territorio en sus diferentes niveles (región, departamento y en sus áreas urbana-rural). Así, el objetivo general fue el determinar la asociación entre la distribución de riqueza y la ocurrencia de la DCM en los hogares desde los diferentes espacios territoriales y en sus áreas urbana-rural en Colombia. Se trató de un estudio epidemiológico de corte transversal (2010), basado en un análisis descriptivo no experimental y predictivo, soportado en los microdatos de la Encuesta Nacional de la Situación Nutricional en Colombia ENSIN (2010). Se determinó la prevalencia de DCM de 11,55% al nivel nacional, en las regiones en un rango de (7,56 – 16,50%) y en los departamentos se movió en el rango de (4,10 – 21,19%). Igualmente se calculó la prevalencia en área urbana y rural con resultados de 11,90% y 10,75%, respectivamente. Se encontró diferencia significativa en la prevalencia de DCM a nivel nacional entre las áreas urbana-rural (X² = 10,742, p < 0.0001) y entre las seis regiones, tanto en el área urbana (X² = 286,752, p < 0.0001) como la rural (X² = 201,044 p < 0.0001). Al agrupar los hogares por quintiles de riqueza se encontró que la prevalencia de DCM a nivel nacional se ubicó en el rango de (8,94 – 12,25%) y fue estadísticamente significativo, en el área urbana el rango se movió entre el q1 y el q5 entre (8,94 -15,15%) y también resultó ser estadísticamente significativo, mientras que en el área rural solo registró tres quintiles (q1 a q3) y el rango se movió entre (8,50 – 10,71%) y no hubo diferencias significativas de DCM entre los tres quintiles de riqueza, la región con mayor prevalencia de DCM fue la Atlántica con 16,50% y la de menor Bogotá (7,56%) seguida de la de los Territorios Nacionales con 9,88%. Al realizar el análisis de riesgo utilizando las regresiones logísticas entre regiones, áreas, y quintiles de riqueza, se evidenció diferencias estadísticas significativas que orientan a que existe en los hogares agrupados en el quintil 1, tienen 1,474 veces mayor riesgo de

DCM que los del quintil 5, que los hogares en el área urbana tienen 1,280 veces mayor riesgo que los ubicados en el área rural y, los hogares ubicados en la región Atlántica tienen 1,799 más riesgo de DCM con respecto a los hogares ubicado en la región de los Territorios Nacionales. Bogotá presentó factor protectivo en la DCM. A modo de conclusión el fenómeno de la DCM en Colombia está presente en todos los niveles (Nación, región, departamentos y en las áreas rurales y urbanas), dadas las diferencias presentadas, de darse la formulación de una política pública ésta debe hacerse de manera conjunta e integral en los hogares con DCM, como en los hogares sin DCM no saludable y que dicha política sea diferenciada por regiones y departamentos.

The Double Burden of Malnutrition (DBM) refers to the coexistence or simultaneity of people with malnutrition (underweight) and over-nutrition (excess weight) in the same population group. It is a public health problem in both developing and developed countries. DBM has been considered as a multifactorial phenomenon, which has been associated with the nutritional transition and the rural-urban migration. It was hypothesized that the prevalence of DBM is associated with the distribution of household wealth and inequality in the territory at its different levels (region, department and urban-rural areas). Thus, the general objective was to determine the association between the distribution of wealth and the occurrence of DBM in households from different territorial spaces and in their urban-rural areas in Colombia. This research was based on a cross-sectional epidemiological study (2010), based on a non-experimental and predictive-descriptive analysis, supported by the microdata of the National Survey of the Nutritional Situation in Colombia ENSIN (2010). The prevalence of DBM was determined in 11.55% at the national level, in the regions in a range of (7.56 - 16.50%) and in the departments it moved in the range of (4.10 - 21.19 %). Likewise, the prevalence was calculated in urban and rural areas with results of 11.90% and 10.75%, respectively. A significant difference was found in the prevalence of DBM at a national level between the urban-rural areas (X² = 10,742, p <0.0001) and between the six regions, both in the urban area (X² = 286,752, p <0.0001) and the rural area. (X² = 201,044 p <0.0001). By grouping the households by wealth quintiles, it was found that the prevalence of DBM at the national level was in the range of (8.94 - 12.25%) and was statistically significant. In the urban area the range moved between the q1 and the q5 between (8.94 -15.15%) and it also turned out to be statistically significant. On the other hand, in the rural area only three quintiles were recorded (q1 to q3) and the range was moved between (8.50 - 10.71%) and there were no significant differences in DBM between the three wealth quintiles. The region with the highest prevalence of DBM was the Atlantic with 16.50% and the lowest Bogotá (7.56%) followed by the National Territories with 9.88%. When performing the risk analysis using the logistic regressions between regions, areas, and wealth quintiles, significant statistical differences were found that households grouped in quintile 1, have 1,474 times greater risk of DBM than those in the quintile 5. Also, that households in the urban area have 1,280 times greater risk than those located in the rural area and, households located in the Atlantic region have 1,799 more risk of DBM with respect to households located in the region of the National Territories. As for Bogota, it has a protective factor in the DBM. In conclusion, the phenomenon of DBM in Colombia occurs at all levels (Nation, region, departments and in rural and urban areas). Given the differences presented, if a public policy is formulated, it should be done jointly and comprehensively in households with DBM, as in households without unhealthy DBM, and such policy should contain a differential component by regions and departments.
