Resumen

La ruptura del ligamento cruzado craneal (RLCC), es descrita como la causa más común de claudicación en caninos. Las técnicas quirúrgicas descritas para la estabilización de la articulación incluyen: estabilización intraarticular o extraarticular y técnicas modificadoras de la biomecánica, dentro de estas últimas se encuentra el avance de la tuberosidad tibial (TTA). La técnica quirúrgica de TTA fue descrita en el 2002, y busca modificar el ángulo entre el plato tibial y el tendón patelar y llevarlo a 90°, en dicho ángulo se inhibe el empuje tibial y por tanto se logra la estabilización dinámica de la articulación de la rodilla. Con la finalidad de establecer tratamientos alternativos con buenos resultados a corto y mediano plazo, y menores tasas de complicación. Esta investigación busco establecer mediante una evaluación biomecánica de fuerza de tensión, si la banda de tensión en 8 es un método de estabilización alternativo para el tratamiento de la RLCC al compararlo con un método estándar con placa; para esto se recolectaron 32 extremidades pélvicas (n=32) de perros adultos; mestizos, esqueléticamente maduros y con un peso corporal entre 20 y 30 kg. Las muestras se dividieron al azar en cuatro grupos de 8 extremidades cada uno, de la siguiente manera: Grupo 1: Banda de tensión en 8/separador tubular (BT), Grupo 2: Banda de tensión en 8/caja metálica (BC), Grupo 3: Placa y tornillos/separador tubular (PT), Grupo 4: Placa y tornillos/caja metálica (PC). Después, se realizó en todas las extremidades una osteotomía estándar para TTA con sierra oscilante, y se utilizó el método de estabilización específico para cada grupo, luego se acopló cada modelo a una maquina universal de ensayos, la cual se encargó de realizar la fuerza de tensión al tendón patelar, y posteriormente se registró la máxima fuerza (en Newtons) que llevo al fracaso de cada modelo y el sitio de fallo de este. Los cuatro grupos se evaluaron con un modelo completamente aleatorizado de 8 repeticiones cada uno. La media de fuerza que llevo al fracaso del modelo fue de: 789.38 N en el grupo (BT), 863.63 N en el grupo (BC), 774.13 N en el grupo (PT), 989.88 N en el grupo (PC). Las diferencias entre grupos se evaluaron mediante una prueba de Tukey. Los resultados mostraron que no hubo diferencias estadísticamente significativas entre el grupo (PT) y (BT). El grupo (PC) mostró diferencias estadísticamente significativas con los otros grupos. En conclusión, el grupo (PC) fue el que soporto más carga de tensión antes de llevar al fracaso del modelo, por otro lado, el grupo (BT) es un método de estabilización viable para la realización de TTA, lo cual justifica su utilización en modelos vivos

The cranial cruciate ligament rupture (CrCLR) is described as the most common cause of lameness in canines. Surgical techniques described for the stabilization of the joint, include: intra-articular or extra-articular stabilization and biomechanical modifying techniques, among these last ones, it’s found the tibial tuberosity advancement (TTA). The TTA surgical technique was described in 2002, and it seeks to modify the angle between the tibial plateau and the patellar tendon and bring it to 90°, in this angle the tibial thrust is inhibited and therefore the dynamic stabilization of the knee joint is achieved. In order to establish alternative treatments with good, short and medium term results and lower complication rates, this research sought to establish through a biomechanical evaluation of tension force; whether the figure-of-8 tension band is an alternative stabilization method for the treatment of CrCLR when compared to a standard plate method; for this, 32 pelvic limbs (n = 32) were collected from adult dogs; mixed-breed, skeletally mature and with a body weight between 20 and 30 kg. The samples were randomly divided into four groups of 8 limbs each as follows: Group 1: figure-of-8 tension band / tubular separator (BT), Group 2: figure-of-8 tension band / metal box (BC), Group 3: Plate and screws / tubular separator (PT), Group 4: Plate and screws / metal box (PC). Afterwards, a standard osteotomy for the TTA with oscillating saw was performed on all extremities, and the specific stabilization method was used for each group, then each model was adapted to a universal testing machine, which was responsible of performing the tension to the patellar tendon, and the maximum force (in Newtons) that led to the failure of each model and the site of its failure was subsequently recorded. The four groups were evaluated with a completely randomized model of 8 repetitions each. The mean strength that led to the failure of the model was: 789.38 N in the group (BT), 863.63 N in the group (BC), 774.13 N in the group (PT), 989.88 N in the group (PC). Differences between groups were assessed using a Tukey´s test. The results showed that there were no statistically significant differences between the group (PT) and (BT). The group (PC) showed statistically significant differences with the other groups. In summary, the group (PC) was the one that supported the most tension load before leading to the failure of the model. On the other hand, the group (BT) is a viable stabilization method for performing the TTA, which justifies its use in live models
